We’d like to inform you that for the foreseeable future, and according to our College’s and government’s guidelines, our office is closed.
We have cancelled all appointments and will call each patient back to re-schedule once we have a timeline of when we will be able to re-open. We will be reviewing each emergency patient on a case by case basis.
Our Medical History Form is available via the link below. You can complete it online and email it to our office. You can also print a copy of the form.
To complete the form online, click the link below, complete the form and click the “Send by email” button at the top of page 1.
If you are using Internet email such as Gmail, Yahoo, or Microsoft Live, when you click the Send button, you will be prompted to save the form to your desktop and then send as an attachment through your email account.
If you are using a Desktop email application such as Outlook, Outlook Express, or Mail, when you click Send, a new email is created in the application with the form as an attachment. You can send the form at that point from your email account.
Note for Mac users: To send form, you will need to save the form to your desktop and then send as an attachment through your email account.
